TeleMonitoring or Remote Patient Monitoring (RPM)

To enhance care delivery and improve patient outcomes for chronic conditions that need constant surveillance, hospitals and physicians are increasing their use of Remote Patient Monitoring (RPM) solutions, which is an in-home monitoring system that collects biometrics from patients and transmits them to a patient management dashboard to be reviewed by their remotely located providers or healthcare team.

Integrated Remote Patient Monitoring (RPM) solutions with messaging and alert systems can let providers know when patients are in stable or critical conditions in-between primary care visits and can help avoid unnecessary doctor's visits through early detection of cardiac or blood sugar health risks.

Remote Patient Monitoring (RPM) can improve the management of chronic diseases by measuring critical risk indicators such blood pressure, blood glucose, weight, etc. Our cellular-enabled devices integrate with peripheral health devices, such as a blood pressure monitor, glucometer, oximeter, or precision health scale. The patient-generated health data (PGHD) is collected, encrypted and transmitted to their physicians and keep patients informed on their health goals. With instant access to patient data, a provider is able to use a clinical suite that categorizes alerts based on importance, generate reports, and view graphs to easily identify significant health changes to make data-informed decisions and determine if new care solutions are needed for said patient. Our portal can host video visits, allow access for multiple medical professionals, caregivers and family to be constantly updated on a patient's health.

Insurance companies and Healthcare organizations, particularly hospitals and Accountable Care Organizations (ACO), are using Remote Patient Monitoring to limit associated healthcare cost, and cut down on the use of more expensive services such as the Emergency Department (ED). Physicians too have expanded their scope of service - and rest assured that their patients are maintaining a healthy status.

MEDICARE & MEDICAID REIMBURSE DEPENDING ON WHICH IS THE PRIMARY PAYER

Effective January 1, 2019, Medicare is now paying for all qualified Medicare and Dual Eligible or MQMB Medicaid patients. For those patients with Medicaid as a Primary payer, you are still entitled to be reimbursed as long as you have an Active Medicaid TPI. More importantly, you can submit your claims directly to Medicaid without first submitting to Medicare, which means you get paid faster, usually within 5 business days when you bill each week.

Medicare (CMS) follows Texas Legislature by encouraging providers to utilize TeleMonitoring service

Medicaid's objective is to reduce costs from patients frequently using emergency medical services and hospitalizations. By authorizing reimbursements for TeleMonitoring services, Medicaid aims to incentivize  physicians to implement this service. TeleMonitoring helps healthcare providers to correct medical concerns with patients before they lead to an Emergency Room visit or hospitalization. This service allows you to see your patients vital signs when taken at their home in real-time from the convenience of your own office. Helping you to improve patient care and change outcomes, and take advantage of state funded bonuses with no added costs or office resources.
